Code of Ethics Complaints

To submit an official complaint, all steps below must be completed.
For the protection of all parties involved, we require that all documents be completed and included in your submission below. All information submitted is held in strict confidence and all proceedings are conducted privately.

1. Read these documents: Before You File a Code of Ethics Complaint (PDF). Code of Ethics and Standards of Practice.
2. Prepare these documents and save them to your computer: Code of Ethics Complaint Form (PDF) Detailed Statement Describing Your Case (DOCX) Any supporting documents.
3. Complete the Code of Ethics Submission Form (below): Complete all form fields. Upload all documents. Click the Submit button. Your submission will be sent to GKAR's Grievance Committee.
